Medical emergencies

The authority of a parent / guardian is required in advance for any medical, surgical or dental treatment emergencies that may occur in the event of serious accidents or illnesses. To avoid delays in organising the treatment during emergencies, if the permit cannot, for any reason, be obtained quickly, it is recommended to give the permission in advance, granting the authority through this consent form to Learn in a Flash Ltd and its agents. I grant permission to the agent appointed by Learn in a Flash Ltd to arrange emergency medical, surgical and dental treatment for the minor under my care if necessary.

Activities - mandatory

I grant permission to the minor under my protection to participate in any activity program for minors that takes place both on site and using means of transport organised by Learn in a Flash Ltd, by the School Houses and by partners.

Media

Photographs or videos can be made to students during their stay and at the host facilities. I grant Learn in a Flash Ltd and its delegates the right to use these photos or video clips as part of future promotional material. Check this box if you agree that the minor under your protection will be part of the future advertising material.

Expenses for emergencies (or agreed by email) - mandatory

I agree that any supplements for food, accommodation, travel or my child's well-being / health will be reimbursed at the end of the stay, such as but not limited to: expenses incurred to allow the child to be repatriated in the event of loss of the travel document or a prolonged extraordinary stay.

Airport and local transport - mandatory

Paid transport must be paid for directly by the student. I confirm that I have read and understood that these costs are possible and ancillary. I also give permission to Learn in a Flash Ltd and its delegates, as well as to School Houses and Work Tutors, to transport the minor for internal travel between School House and workplace and lessons and other places.

Unsupervised Leave Permit - mandatory

I give permission for the minor under my care to leave the workplace / host family without supervision during their free time. I explained to the minor that he or she must respect all the rules of the School House and must return in the evening at the established time: 22.00 (10:00 pm)

Acceptance of Terms and Conditions on the website www.learn-in-a-flash.com - mandatory

I declare that I have read, understood and fully accepted the terms and conditions ("Terms and Conditions" on the site where parent and student enter their data to be able to participate. I accept the insurance conditions and the cost of interventions not included in the service (not attributable to School Houses, to Learn in a Flash Ltd and local agents) £ 20 fixed costs and £ 30 per hour with a maximum of £ 150 per day; some practical examples: assistance with the recovery of a temporary travel document in case of loss, remote and on site assistance in case of offences and so on.

Amendments

If you wish to change the decisions on this document, you will need to send a new document by registered letter. Those indicated as such are mandatory.

Privacy:  Where Learn in a Flash ltd receives any personal data (as defined by the Data Protection Act 1998 and following relevant), it shall ensure that it fully complies with the provisions of the Act and only deals with the data to fulfil its obligations. In fulfilment of its obligations under the Act, Learn in a Flash ltd as the party providing data should too, shall have such systems in place to ensure: a) Full compliance with the Act b) In particular, compliance with the Seventh Data Protection Principle which deals with the security of personal data c) The reliability of all its employees who may be involved in processing the personal data. Parties involved shall take all reasonable steps o ensure that all its partners contractors and agents comply with this clause where they are processing any of personal data. Other parties involved shall allow Learn in a Flash ltd reasonable access to such information as is necessary to ensure that it is complying with the agreed and legal provisions and the Act as a whole.
